What do you need to bring to NMC when seeking health care?
  • Most recent pay stubs (Number of paycheck stubs required depends on your pay schedule)
  • Disability statement
  • Current Photo ID card (Driver’s License, Passport, etc.)
  • Current child support statement
  • Current food stamp verification
  • Current unemployment verification
  • Insurance Card (Medicaid, Medicare, BCBS, etc.)
  • Proof of residency (Voter’s Registration, Vehicle Registration, Utility Bill, Lease, etc.)
How much does it cost?
